A copy should be retained by the donor and given to the Girl Scout who requested the donation. For official acknowledgment and tax deductibility please also send a copy of this form to the local fund development office. | Incident Report Form | Fax or email to the program assistant in the Oakland office | Use to report any non-injury incident that may result in legal or other repercussions for the people involved or for Girl Scouts of Northern California including adverse media |
| Media Information Sheet | Give to media personnel if an emergency should arise where media arrives to cover a news story. | Event Directors should download several copies of this document and bring to the event. Consult with the council emergency team and law enforcement (if applicable) to fill out the information about where media can go and with whom they may speak. |

| Permission to Travel Form for Minors | Trip Leader | If adults are traveling with minors across international borders, they may be asked to produce this form (signed by a notary public) in order to allow them to cross international borders with children who are not their own. |
